For Mac users, this is surprisingly simple using Garage Band (part of the iLife product line).
Simply take your source audio material (no more than 30 seconds in length), and plunk it into a new project in Garage Band.
Then, go to "Share" > "Send Ringtone to iTunes"
When this is complete, simply connect your iPhone, go into iTunes and from the "Ringtones" tab under your device enable the new ringtone.
Do a sync.
Finally, go into iPhone settings and choose your new ringtone from the sounds menu.

CAN'T HEAR MY PHONE RING WHEN SOMEONE CALLS ME I HAVE TO PUT THE PROFILE ON VIBRATE

Each of your profiles can be set to different ring tones and different volumes, so whatever profile you have your phone set to, make sure you have it on a ring tone you can hear, and make sure the volume is turned up. Let me know if this doesn't work.

This is used to set up the different ways of incoming call prompts in various situations. Use

Using a music file as a ring tone

Go to Tools->Settings->General->Personalize->Tones. Or wherever Tones can be located in your model. Then go to Ringing Tone and select the mp3 that you want. You can even set mp3s for other events like on receipt of msgs, or rintones for different contacts, alarm and emails etc.

The phone doesn't ring, there is no speaker phone, and no tones when you dial or text.

Try using the side keys to adjust the volume. It sounds like the phone is on silent. Go into the audio settings and play around the ring tone, text, dial tone sounds and settings until you feel that it is appropriate for you.

If the phone is still not responding with sounds or tones after you have set the audio settings, the phone might be defective and you may need to have the phone company look at it.
