Hi,
The PUK code is only available from your Network Provider. PUK is a personal unblocking key which is actually related to the Sim card and not the actual phone itself.
The good news is the Network provider will give you the PUK for free.
All you need to do is ring your provider (on another phone obviously) and once they have established that you are the owner of the phone they will give you the PUK for free and directions how to enter it into your phone.
