Problem is not with your modem and i can assure you of this fact.
This has occured because your service provider perhaps changed the APN or some other port or setting. You need to verify this from your service provider.They will help you to configure your modem
As modem is showing signals there is no problem with your device.
Try and use these settings
Account: AWCC GPRS
Connection Type: Phone Line (GPRS)
If these don't work verify them by calling costumer care of your service provider
