Since it's Phone Lock, service provider will not helpful. I hope you yourself changed the code and have forgotten. This is very common, but the solution doesn't work for all. There are few tips,
2. Unlock using Nokia Flashing software
3. Unlock using Nokia Updater.
Formatting has several methods, using the button or using codes. (I can't guarantee this will definitely work for you, but it's worth trying and there's no cost either).
(will loose everything saved on the phone) On the main menu, type in *#7370# and watch what happens, if you are asked to enter a code, then type in 12345 or anything you want. [type in the code as you type in numbers before dialing a number].
Another code is (not actually formatting, but hard factory reset), *#7780# with the same procedure as mentioned above.
The second method is using unlocking software like UFS box or Jax box. This is 100% and there's not side effect either. But to do this, you have to visit a nokia service center or Nokia Mobile repairing Shop. The hardware and the software cost more than the price of your handset, so don't try to purchase, rather get it unlocked from them (if other methods don't work).
The third is, formatting the handset by pressing the buttons on the handset.
First switch off the phone, [if there's contacts, smses, saved on your phone, everything will be lost. If you already did a backup, then you are saved from this doom..:-)]. While the phone is Off, press * + # + Power button (press *, # and power button simultaneously)
You'll need to confirm any warning, simply press "yes" or "Ok" and proceed.
The last method is using the Nokia Updater. I hope you've heard about this, but this is risky if done without patience or incorrectly. Once failed, then the only solution is Flashing, no other way to get it worked again.

Download and install the Nokia Updater, [you'll need to restart your computer after it's installed]. Now connect the phone using the usb cable and select the connection mode as "Nokia Mode" or "USB Mode" and open the Nokia Updater on your computer, it'll detect the phone and will give you the step by step onscreen instructions on how to update the firmware. Please note that this is to be done while the computer is connected to the internet and while doing so, the connection should not be fluctuate or down. Don't every pull out the cable or don't every touch the phone/cable while running, keep the phone on a stable position and start.
This are all the methods to unlock your phone's security code. If all the manual methods don't work, the only and final solution is Nokia Mobile Repairing shop or Nokia Service Center.
