Check the voltage on the battery contacts, check the charging voltage on the battery terminals of the phone,if low replace adaptor.
If Ok then replace battery and charge, if the battery does not retain the charge then replace battery as it might have developed reverse polarity. If adaptor is ok but charging voltage at terminal is low , check the mother board.
