Question about Samsung SGH-D500 Cellular Phone
YOur SIM-card is blocked. This means there is no general code for your phone model, but the PUK(Personal Unblocking Key) is unique for your SIM-card.
Some service providers provide the PUK with the card. In any case you should be able to get it by contacting your service provider.

Tips for a great answer:

dont try more than 3 times.If you try more than 3 times the Sim will get
locked.And it will ask for PIN code.Both these PUK and PIN code can be given
only by the network service provider. Please call the network service provider
of your mobile.And ask him to provide the PUK code or PIN code.He will ask you
some basic details like name,date of birth for his verification.And he
will provide you the PUK and PIN
code.This is done for free of cost.
