Question about Cell Phones
Possible that the battery has lost the charge retaining property as it is weak due to reverse polarity .
Check your charger voltage and the voltage at the pins of the phone contact with the battery removed and adapter connected. If the voltage is reading good then it is confirmed that the battery needs a replacement with one that is genuine with warranty.(not old but a recent date of manufacturing is important.)
