First of all, get some toothpaste and a soft rag, try cleaning the disk thouroughly, sometimes the scratches on the disk can be so small that they aren't too visible, with this cleaning, you can fill these tiny scratches and allow the disk to work perfectly. If this does not fix your problem, take a Q-tip and run it along the inside of the disk tray while open, if you brush the bottom very gently you can successfully clean the eye that reads your disks of any dust and or obstructions.

Hope this helps :)

Where can i find the eye of truth

To acquire the "Lens of Truth" you must return to the Temple of Time and replace the Master Sword, thus returning to Young Link. Then, travel to Kakariko Village and enter the Windmill in front of the well. Talk to Guru-Guru, the man furiously playing the spinning music box. If you don't know the song of storms he will teach it to you. Play the song of storms to make the windmill spin rapidly which drains the well. leave the windmill and enter the now unsealed well and you will arrive in a mini dungeon called the "bottom of the well". The Lens of Truth is inside this dungeon.

N64's Zelda, majoras mask problem

The Jumper pack is needed for Majora's mask, as the extra space is needed for the game to play the enhanced graphics. Ocarina of time was made before the jumper pak was introduced, so it does not need it.
As for installing it, get a jumper pak installer kit to remove the original memory pak, the insert the new one.
