Possible that the pads are worn out and hence is not contacting well. The second issue is a serious one and I think it is important that you will need to contact Nikon service as it is not a DIY job.
The fault can be that the servo is faulty the controller is faulty or the drive is jammed.
If however your camera works in normal mode then I advice to use it well tilll it lasts than to look around for spares as the camera is 5 years old.
