SOURCE: I've got a 3580 Ultrium

I had this same problem a few mins ago. I held the tape up with my finger as I hit eject. The tape came right out. Then I put in a cleaning tape and had no issues at all. You don't need to disassemble the unit at all.

SOURCE: I have the following equipment. Tape

Hi, I am steve, lets get you some help.
Sounds like you are having a major problem with this system. My recommendation is to perform a fresh install of the tape unit. This is a lot of steps so the best option is to follow the device manual
http://publibfi.boulder.ibm.com/epubs/pdf/a67ru3ct.pdf
The reason to use this approach is to remove any corruption in the system that may be causing the error. Once is has been installed as new we can determine whether the unit is faulty.

SOURCE: Why IBM Ultrium 3 lto

Hi well the TD interface circuit may have failed?
IF the device isn't even "Recognized" and ISN"T listed in Device Manager, then it is most likely faulty, I am sorry to tell you.
SCSI is a very robust format, and the ONLY time a condition like this can exist is if the device is faulty.
These TD indeed fail in this way frequently, and I have replaced many units over the years, with exactly this "Fault" condition, this is "Classic".
The only option you have is to try to replace it with the same make and model, IF you can indeed obtain one?

IBM 3580 Lto3 drive showing in Device manager but not showing in ntbackup while taking bkp

I am guessing you don,t have a driver installed for this device.
Just manually add the driver but look for LTO as the MANUFACTURER - there is a generic driver in there which should work fine.
Better still, also upgrade your backup program - even to an old copy of backup exec. It has proper cataloguing and also its own driver family too.

Looking for documentation on the jumpers on the back of the IBM LTO 3 (39M5657) tape drive. Thanks!

the jumpers most like to set SCSI ID, you can just jump it and check if the ID changed from host, it doesn't need a document usually.

Stuck tape

The tape won't come out when resetting the drive. Error 6 is serious.
To get out the tape you must open the drive. But IBM has a discription online (and in the user manual) how you can do this.
