If you try pin(PERSONAL IDENTIFICATION NUMBER) more than few predefined no of times your phone will ask PUK code which can be given only by your service provider which is specific for each and every mobile number .
Donot try PUK code many times or else your sim will be blocked and you will need a new sim.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no .
Go to the nearest service outlet of your service provider they can also help you.
There is no other go than the above procedure only your service provider can give it nobody else in this world nor any software.

SOURCE: i lost mind my puk
Please call the Customer Care of your Network Operator (sim card provider), they will tell your the code. There's no other way or no means to get the code except from the Service Operator. They might ask your few questions related to the Sim Card, like your name or your address, tell them promptly and they will give you the puk code for your sim card. It's also important to note down the Sim Card Number (number written on the sim card) if in case they ask you the sim number. You will also be assisted on how to disable the security settings.
Note: if you keep entering random/unknown numbers/codes, the sim card will be locked and will be rejected. It'll not be usable anymore. Good luck.
Thanks for using FixYa.
