HI! THE ONE QUESTION I NEED TO KNOW IS WHAT OS? JUST GENERALLY I WOULD SAY THAT YOU MIGHT TRY TO UPDATE THE OS TO THE LATEST VERSION. AGAIN, NOT KNOWING THE OS, THIS IS A FIX I WOULD TRY. YOU SAID THE PROBLEM HAPPENED AFTER YOU INSTALLED THE OS, BUT WHAT ELSE WAS INSTALLED ALSO? YOU MIGHT NEED TO LOOK AT ALL PROGRAMS TO BE SURE THAT YOU HAVE THE LATEST VERSIONS AND THAT THEY ARE COMPATIBLE WITH YOUR OS. SO, THESE ARE SOME IDEAS FOR NOW. TRY THEM AND IF NO LUCK, REPLY AND LET ME KNOW THE OS YOU ARE USING AND THE PROGRAMS INSTALLED. OK? I'LL BE GLAD TO HELP.

Well, I had the same problem, so I open the control and clean with a little alcohol or acetone the panel inside. Now I got my Koss control working 100%

I would back off the 9500 GT and get the onboard video working first.
I would also suggest that you don't have a good enough PSU for that 9500 card, Corsair memory, and the and the AMD 64 x2 5200.
That could account for the slowing down and occasional blank out. The Extreme 450 is much better suited for a milder, stock system.
For that system I would suggest a GOOD PSU at least 650 and preferably 700 to 750.

Since you have narrow down the problem to be the monitor, the next to do is to open it up. First place to look will be the capacitors in the power supply (common problem with ALL monitors). Please look here to get some ideas what to look for inside.

Hello dimash, you must remove nvidia old drivers before installing latest driver, go to Start> Control Panel > Uninstall a program > Nvidia Drivers Remove, Restart Computer, i found newest drivers for you video card, which operating system did you install on? To Change Resolution Go to Nvidia Control Panel > Display > Change Resolution > Color depth : Highest (32-bit)
Click save driver to your computer, install the driver, don't forget to restart the computer. Here is the links.
