The problem of PUK number or code is not simply a phone problem but rather it is a SIM problem. Probably you must have entered a wrong PIN number severally which will now make the phone to request for PUK number or code and it is usually written on the SIM packet but if you can find it, just call the customer care of the network you are using and request for the PUK number, they will be in best position to give you. else you won't be able to use that phone unless you put another SIM card.
