The PUK comes with the SIM card - its an individual number, so no one at FixYa can give it to you. However, if you provide proof of purchase to your phone service provider (T-Mobile?), they can unblock your SIM or provide you with a new one. With a new SIM you'll keep your current phone number but will loose any address book data you stored in the SIM.
