It sounds like the part the toner bottle slides under to lock in place is broken. you could try and fold some paper and tape it to the toner to hold it down.

Did you follow the instuctions to shake gently then remove all tags prior to placing the toner? Is it a toner/drum unit. look underneath if theres a green drum. After market toner will not work.

Take the unit out and install it again, making sure it is in all the way and sitting correctly. Then reset the machine and see if the error goes away.

SOURCE: I need to know how
If I am not wrong, you are looking for information about how to reset the WIM (web interface monitor) password. Actually, the password and the username is saved in the NVRAM (Non-volatile Random Access Memory) of the machine which is a tiny computer chip pre-installed on the motherboard. There is only one option you can try instead of re-seating or replacing NVRAM. I hope you haven't tried the "supervisor" account, if not, try logging in by entering the username as "supervisor" and leave the password blank. If this works, you are saved from replacing the NVRAM and or calling a local tech to do it for you (and of course you are also saved from buying the NVRAM chip). But if in case the 'supervisor' account login details has also been changed, then you don't have another choice instead of replacing the NVRAM. I am sorry but your only option is to contact Ricoh or to call a local tech to get it done for you. It's not recommended to do it yourself since a slight mistake might damage the machine permanently and also you'll need to know which type or number of NVRAM is installed on your machine's motherboard.
Let me know if you need further help.
Thanks for using FixYa.
