It sounds to me like something may have been accidently changed. Double check bobbin is clean and in correct position. Also see if there is a thread caught in one of the tension guides, by pulling a thread carefully through the machine towards the needle.
Rather than breaking needles, test the tension by putting foot down and pulling on thread by hand , if its too easy to pull through something is causing the tension to be too loose

Why does it knot up underneath?

If your machine is creating a mess of thread under the fabric, the upper thread tension is too loose and is causing what is called "thread nesting" or "bird nesting." Tighten the upper thread tension (righty-tighty) and test. Keep adjusting until the upper thread and bobbin thread meets in the middle of the fabric.

You might also try this method as some machines will create thread nests when the first stitches are made:

My tension is too loose, have tried adjusting the dial and still cant tighten it. have a brother fashion faces machine lx 3125. it is relatively new three or four years old not much use

Check the upper tension discs for lint: Raise the presser foot to open up the discs and blow or 'floss' between them. Any lint or thread in there will prevent them from giving the proper tension.
Good luck!

My tension is way off. The machine is about 35

Set the tension to 4 and then try this - thread the machine with the presser foot up.

If this still does not work, call the service dept and tell them they missed something when they worked on it before - have your receipt at hand.

Can not adjust the tension

Nothing is too old, that is what the internet is for. You can google it or you click this link (PFAFF Manuals) which I found for you. Just look for your model and easily download the owners manual after purchase. Don't lube any part of your machine until you have a manual which shows you where. Oil attracts lint and could cause it to operate in a difficult manner.

Help! Tension Problems

Looping on the top means there is no tension on bottom. The bobbin thread generally rotates counter clockwise. The thread must get under the tension spring. The spring must be adjusted equal to the top tension. This you will have to play with to tighten or loosen as needed. When you sew in a zigzag you might want to loosen the top tension slightly. On a regular basis remove the bobbin case and clean with a qtip. On the race put one drop of sewing machine oil. Clean the bobbin case and replace.
