Usually focus problems comes from the monitor and rarely from the display adapter. If possible, check your monitor by connecting it to another PC or laptop and try to use the same resolution and refresh rate as on your system. If the same fault appears then you should contact a service for the monitor repair. Another way of testing is to start your OS in safe mode by pressing F8 key before the OS starts booting and select Safe mode. If image will appear correctly then it could be the graphic card's fault, in that case try to reinstall the video driver or use another card.
