The most likely possibility is that the dimmer simply broke. I would call Lutron tech support at 1-800-523-9466 to help you diagnose it and possibly send you a replacement (available 24/7).

At the switch you have a common ( either a hot or a wire going to lt). And two Travelers( Wires going from one switch to the other) Make sure your not mixing one of the travelers up with the Common easy mistake happens all the time. Travelers are always in the the same at the other end of the 3w switch , you may need to take it apart and look. If you need further help send me more details I hope this helped

This is not a difficult job. First of all go out to a home depot or other hardware store and purchase one three way switch. They come in white, cream, and brown.
Back home, kill the power to the lights you are working on. Then open up the dimmer switch, the other switch doesnt need to be changed. Now, simply draw a diagram of where each wire color is now connected and remove the old dimmer. Reconnect the wires. Remember that the white wire goes on the dull color screw and the other two go on the brass screws. Turn the power back on.
If you do all this and the switch only works correctly part of the time just reverse the two wires on the bright brass screws. Please mark this answer as a fixya!

There is one more thing that you can try, turn the power off from the breaker and wait about 20 seconds, then turn back on. If that doesn't fix it you are probably right about the dimmer being bum. It is very rare that Lutron devices are defective. You can contact them directly and see if you can get it replaced since you no longer have the receipt. You may have to send it to them before they will send you a replacement but it beats spending another $50
