Question about Sperry Instruments HST401 Voltage Continuity Tester
If the bulb is a regular and typical flashlight bulb, then a simple resistance measurement across the bulb will tell you your answer. If you get resistance, the bulb is good. If not, then the bulb is bad (broken filament).
The manual for your 401 is here. Follow the instructions in it (second column) for checking continuity of an unpowered device.
If your tester doesn't appear to be working as it should, then it likely is time for new batteries, or it has been exposed to a voltage higher than 250vac, and is damaged.
