Hi your only option is to call your phone service provider, I've explained more on this page http://www.bukisa.com/articles/19406_puk-code
Theres really noting else that we or you can do about it.
its a random generated number by your service provider and .. how do you guess a random number?
Let me know if you need anything else.

Just call your provider and tell them you need a PUK code, they will be more than happy to give it to you.
you may also get it from the company website or go into the provider store.
this is something they do all the time

If you log onto your companys website under your account and go to the option that says devices you can get the puk code- If you enter in too many numbers it will permanently block your sim card and you will have to buy a new sim card from your cell phone company- they are about $20

Do not enter in any codes if the phone asks for a "PUK" or "SIM lock" code. Contact your cellular provider immediately and request the code because if you enter in an incorrect code, your SIM card will permanently deactivate itself.
