SOURCE: phone locked need PUK code
Unfortunately, your going to have to call the Manufacturer or your phone to solve this issue. The Manufacturer of your phone would be Nokia, LG, Samsung, Motorola. They are the only ones who have access to this information. Some places may charge you to get the unlock code but most won’t so it’s really hit and miss. Nokia charges 14 dollars. In this case you need to contact. If your looking at your phone screen and it’s asking you for a security code, you may want to try the last 4 digits of your cell phone number, that is the default for most phones. If that does not work then you can try 9999 or 0000. Those are also standard with most phone companies. If after that you still have no luck, call your service provider to get help with the code otherwise you risk locking your phone out permanently which stops you from being able to receive phone calls, text messages, etc. I’m sorry your having so much trouble with this! I hope I was able to help you today!

If you need the PUK ( Personal Unblocking Key ) for your phone, you will have to contact your mobile phone service provider.
PUK code is related to the SIM card you are using. So, kindly call the Customer support of your mobile service provider and they will provide you with the PUK code.
Also, entering a wrong PUK code ten times in a row disables the SIM card permanently. So, please be careful while entering the PUK code.
Hope this solves your problem. Thanks for using FixYa.
