There are two possible reasons for this problem.
The easiest to fix is calcium build up blocking the shower head. Remove the head and soak it in (for example) LimeAway or CLR, then rinse it and flush it out very well before reattaching it and, especially, before trying it out.
If this doesn't get results you likely have a problem with the washers in the diverter (that thing you pull to switch to shower). One of them may have deteriorated to the point where it worked its way into the shower line and is blocking it.
Are you willing and able to disassemble the plumbing to make the repair?

SOURCE: water only comes out shower head, won't come back

sounds like the diverter is jamed. it may be in the spout or a button in the escheuchon. if spout try to unjam the pull up knob or the pull down ring. if in the escheuchon remove part and replace.

SOURCE: Ahhh... I bought an older

Normally the diverter in the spout does just that. It's basically just a little door you pull up to stop water from flowing out the outlet to bathtub, and it diverts water to shower head as it has no where else to go, as the door has been shut. Think of a "T", if you have water coming in, it's going to go out both openings if nothing stopping them, and they are at same level, but when you plug lower one, the water flows up and out the other. My guess is your getting very little pressure from head when it's still coming out at tub.
NOTE: before attempting this, make sure you incoming water supply to house is shut off at valve. Anyway, those spouts are usually held on by a set screw up under bottom side of faucet near back of it, (close to tub) there should be a small slot which will probably be an allen head or philips. You back that screw out, then twist and pull and it should slide off of copper pipe. If no indication under there of a set screw then there are also those that just thread on, which requires you just turn faucet counterclockwise to remove. Replace entire piece all in one, and be done with it. Most hardware or home centers will sell just that separate. Make sure you get the one with diverter. (handle that you pull.) Clean pipe well with steel wool if it's dirty and corroded before sliding new one on, and wipe some "vaseline" on pipe to help and protect "O" ring inside spout. Slide new on on all the way and tighten set screw on underside. If threaded, clean threads, add pipe dope or teflon tape and thread it on until aligned and tight. Good Luck.

it is full of lime and scale from mineral deposits.
either chip it away to clean or remove and replace the spout with a new one which is the easiest thing to do instead

Low water pressure from shower head with the shower head removed

If you have good pressure at the water faucet close to the shower and low at the shower then the shower valve needs rebuilt. If you have low pressure at all then start going down the line toward the water service coming into the house looking for where the low pressure starts in the distribution system and you should be able to isolate the problem.

Water comes from both the shower head and bath tub faucet when bath is turnd on and wont turn off when shower is on

if it is standard shower where you pull the knob up when water is running. the pull knob lifts a flap to shut off water to bottom faucet it proably has some buildup on the flap clean well with some clr or other calcium cleaner like it so the flap gets a clean seal on downspout

My 3 handle bathtub fauset [price pfisher] cycles hot and cold

You will need to either replace center "shower diverter"assembly or a quicker fix would be to remove old tub water fill spout and replace with a "pull up diverter knob" type water spout. Cost about 10 dollars Hdepot Pull Knob to divert water to shower when showering.

No shower water

the spout is also the diverter which sends the water up to the shower head. Assumming that the shower head isn't clogged with lime you should be able to solve the problem by removing the spout and taking it to a hardware store or home depot for a replacement. Most spouts just unscrew but some have a set screw which holds em in place. Look for the set screw on the underside of the spout. good luck
