I apologize for the late reply, yet I believe such details should be asked to your phone company. They usually supply you the PUK code along with the PIN code for cases such as these. If you lost those codes, then it's best you contact them and ask for it. You will probably have to verify yourself as the owner of that phone number before proceeding.
