As i understand correctly, you canot make the modem work with windows vista, right?
Make sure that the OS Window Vista has not encountered any problems while installation process.
Try to create a conectoid
click start>control panel>internet connection>network and sharing center>setup a new connection or network>connect to the internt>broadband PPPoE> Enter you User ID and the Password (contact your ISP) confirm the password and click connect.
You should be conected.
Make sure the ADSL LINK or DSL light on the modem is solid before you create the connectoid.

check that the pci slot is nice and clean then make sure the card is inserted fully, and restart your machine, if the problem still persists try and get the most up to date driver for the card from belkin's website copy following link and paste in your address box
http://modem.free-driver-download.com/D-Link/14620/D-link-DFM-562IS-Modem-Driver-Windows-XP.html and install it restart the computer if the problem is still there it's 100% faulty modem card and it needs replacing.
