If all 4 lights are flashing red it means it is not recognizing the audio video cable. Try unplugging and replugging the audio video cable at both ends and then try restarting the system. If this does not work try a new cable (cheap solution) failing that it may be an odd hardware failure so you'd have to contact M$. Hope that helps

hey there, I would open the Xbox 360 dashboard, then go to memory, and press Y on the hard drive to open the "Device Options" menu. Then press the following sequence of buttons: X, X, LB, RB, X, X. You will be asked if you want to perform maintenance on your hard drive and delete all patches.select yes I hope this helps
Best advice is to contact Rockstar, they might have had simular complaints about the game, the proof is in the pudding though, basically proving that it was the game that destroyed itself and not a defective console that did it, it also contacting MS to see if they had complaints.
From what you are though, it sounds like the laser is being told to get to close to the disc, causing the grinding noise and the groove in the disc.
I am almost 100% certain that the warranties of xbox 360 consoles have been extended 3 years.
I also had this problem some months ago, and Im sorry to say that I was told the problem is in the hardware. I had to send my xbox in, and they sent me a brand new one, fully covered under warranty.
Give xbox a call... 1-800-4MY-XBOX, ask about the warranty of your system, and also let them know your problem. The only way I ever got any games to work with this problem, was to open and close the tray over and over until it finally worked, so I finally just sent it in. Good luck.
