This is a pretty easy fix, you need to remove the back, and locate the cable, either join it to another lead, or resolder it on.. you can get cable socket etc, from say Radio Shack or the like.

An AC adapter has no polarity. Are you asking what the DC output is? That would be written on the adapter itself.

I had to purchase a new adapter from radio shack.

find a replacement ac adapter on ebay or amazon it should work on variable currents and amperes. And it should function just fine. Or go to radio shack and buy a 12v adapter that has different tips that you can switch around and find the right tip and your off. Ok, I know this solution is a little too late but tell me if it was still able to help you.
