The scratch was probably a ring around the CD, right?
You can try a resurfacer on it, a GOOD one, not one of the 10 or 15 dollar things. This sometimes fixes the problem, but not always. If it doesn't, then your going to have to replace the game :( Sorry.
As to why it did this. Often this is caused by the system being bumped in some way while the game is spinning in your console. Sometimes if the system gets bumped it pops something loose to the point that it no longer needs to get bumped to cause this ring scratch on the CD. Only way to get this fixed (without voiding your warrenty) is to contact microsoft and ask them about repairing it. If you ARE still under warrenty, they might even replace the game for you.
