Ok, if you do not want to call yur service provider, you need to have a look at the origional packaging you got your sim card in.
Your puk code will be stated clearly. If you do not have this any more, you need to phone your provider. Have a look at THIS link.

since your sim is blocked thats why you require to contact service provider. You can't use your phone to call your service provider. You have to call them through other sim card of the same provider or ask the vendor to get the puk for you from whom you purchased your sim card.
