This problem is very common and most users think that it's related to phone, but in fact it's not, this is only with you sim card and if you keep entering random codes/unknown numbers several times, you sim card will be rejected and will not be usable.
Your only solution is to call the service provider (Network operator's customer care), as them to give you your puk code, they'll promptly give it to you after few inquiry to make sure you are the real owner of the sim card.
There's no other alternatives, or not other options that you can get the code, so don't play around, rather make a call to the customer care of the sim card network operator and get the code.
