PUK code is belongs to your service provider you need to contact your service provider customer care toll free number and they will ask for your mobile number and sim card no you need to revel that and then they will provide your PUK code

If you try pin more than few predefined no of times your phone will ask PUK code which can also be given only by your service provider which is specific for each and every mobile number . Donot try PUK code many times or else your sim will be blocked and you will need a new sim.
Contact your sim customer care department over phone. Its free of cost to get the pin or puk no.
Go to the nearest service outlet of your service provider they can also help you.
There is no other go than the above procedure only your service provider can give it nobody else in this world nor any software.

puk code has to be given by sevice provider , you have to call to your service provider ( sim company ) and tell them the sim number then they will give you PUK code

You will need to contact your service provider for the unique PUK for your mobile number. You have three attempts to enter SIM PIN number then after three incorrect entries it will request PUK code (like you have at the moment). Please dont keep guessing as you only have ten attempts at PUK request level! If you do it will lock your sim which will then require replacing by your network! (at a cost usually?)
Some networks like O2 in the UK will allow you to get the code from their website (search PUK on site search).

It's not the problem with your phone, contact the customer care of your service provider (network operator), and tell them your problem, they will give you the number instantly. If you enter wrong/random numbers several times, your sim card will be unusable. Good luck
