both of these codes are provided when you purchase a sim card and are usually written on the sim holder or on the provided booklet. if u have the puk code, then u can enter a wrong pin multiple time and then the sim card will ask you for the puk code which then u can use to unlock the sim. if u lost both the sim and puk, then the only way is to contact the network operator to give you a new sim card.
