Call your network and they will give you your PUK code.
They are the only ones can do this. Get them from the internet and you will more than likely block your sim...PERMANENTLY!
They will ask you for your phones IMEI no. You will find it under the battery or if you still have the box it should be on that.
Then they can tell you the code to unlock that handset.
