The "gears" are simply to restrict the handle from turning to too hot a setting. Mostly if you install them lined up to the top you'll have maximum handle travel to the fully hot setting if that's what you want. Remove the shower head and see if the flow from the pipe is strong. If it is then the blockage is in the head itself. Check for buildup in the screen and clean it out. There is also most likely a flow restrictor in the head which can be pulled out with needle nose pliers. If the cold runs OK and it's just the hot that's slow you may need to clean the cartridge in whit vinegar to free up the pressure balancing slider.
