You have to go to you service provider and tell them them to fix it just tell them the kids got a hold of you phone and this is the outcome then they will ask for your number and some other info then they will unlock it and then you can change you pass code

if the PUK is forgotten it will not help, you better change a sim in orded for you to use your fon, because if you kepp trying the wrong code it will SIM CARD REJECTED
