If you forgot the password of the facebook. Simply go the facebook link forgot password.

Simply give the mail iD( which ou have given to the facebook while opening the account) and and answers to the security questions and then they will sned the password to you ans login with that password and change the password as it has expiry time.

Sounds like a suspicious request to me. If you can contact the admins from another computer I'd try that. The survey thing has all the hallmarks of being an attempt to gather ID, possibly for dodgy purposes. I would NOT respond to it. You might also have had a hack attempt

it is most likely one of 2 things. Your account wash phished or stolen and used to gain access to facebook credits for surveys. And you had to do the am i a human survey, which is like select blue, whats 10x3,etc. Or you have the virus which is blocking your facebook this virus can be seen on youtube for demenstation purposes. But basically id run malware-bytes anti malware software and a antivirus in that order. To ensure you do not have the malware virus that causes this.

now if you took the Am i Human survey and didnt answer it correct and are blocked. You need to email facebook and read this http://www.facebook.com/help/?page=174 read the maintaining your account section.
