Get PUK from Service Provider...

PUK code is not within the phones, meaning it is not related to phone security, PUK code is a sim security, you can get the PUK code with the SIM bed or the tray where the SIM was attached when you bought the phone, also you can get the PUK code from a security form also included in the box with your phone.
If you cant find these anymore, you can drop by or call your network provider for they may have records of you SIM and PUK codes.
