There is service bulletin that says:
Measures against noise, E014, and faulty fixing caused by coming off of fixing roller
Detail:
<Symptom>
The fixing roller comes off, leading to noise, E014, and faulty fixing.
<Cause>
Because the fixing spacer [2] does not rotate while the fixing roller [3] is rotating, the retaining ring [1] becomes
detached due to frictional resistance with the fixing spacer, and the fixing roller comes off as a result.
<Factory Measures>
A projection (P-2 [1]) is added to the fixing spacer. When attaching the fixing spacer, fit the projection in a
groove (P-3 [2]) of the fixing roller shaft so that the fixing spacer will rotate together with the fixing roller and
frictional resistance between the fixing spacer and the retaining ring will be eliminated. Therefore, the retaining
ring is prevented from becoming detached.
Service Work:
Replace the fixing spacer with the new-type fixing spacer when the foregoing symptom has occurred in the
field.
To prevent occurrence of the symptom, check the shape of the fixing spacer when replacing the fixing roller,
and replace with the new-type fixing spacer if the previous-type fixing spacer is attached.
<Point to note on attaching the fixing spacer>
Fit the projection (P-2 [1]) in the groove (P-3 [2]) of the fixing roller shaft to attach the fixing spacer.

This is factory error, and I think that you must call service and have it fixed under warranty.
