when you have your document opened, look every layer to find little yellow triangle(s), witch means that there is a problem with this(those) layer(s), it can be (more often) the text layers that have fonts that you don't have installed, or a reference file that has been moved from original path.
hope you'll solve that !

I've never heard of this, unless the user is saving to a PDF format. Have you tried saving a Photoshop document in a different format? Photoshop files are: PSD's or PDD's. I have only ever seen 'presets' in PDF format. In CS3, these are set by selecting: Edit> Adobe PDF Presets.
1 choose the filter mosiac from the filter menu.
2 choose any options form your window
3 choose "ok"
in your special filters you will find the following
Pixelate: Color Halftone, Crystallise, Facet, Fragment, Mezzotint, Mosaic,Pointillize.
of course you will choose Mosiac.
you may want to down sample your photo to 300 dpi
then adjust your brightness and contrast to your desired likeness.
You can also change the grayscale doing this.
