You have to contact your service provider of your SIM card to get the PUK code. You have to give your personal details to the service provider to get it and also the sim number which is on the backside of your sim. Dont try guessing your PUK code because wrongly guessing for 14 times or so can deactivate the SIM card and then you will have to replace the SIM card
