If you don't remember the password you can, of course, reset it but you have to know which email address you used to setup the PSN account.
If you don't know the PSN account email address, you'll need to go to the web site on some PC with a browser and go to the account settings there to see if you can provide other information that identifies you to either let you change the email address/PSN ID or have them show you.
Not much to go on, and you'll need to do the legwork on the PSN web site outside of the PS3 console until you figure it out and then you can go to the console and sign in with the refreshed credentials.
This could either be a problem with your internet or a problem with your browser.
I'd say delete all cookies from your browser and then attempt to upload again.
Also, try and reset your internet connection and then attempt to upload again.

I think you may need to go to tolls and delete cookies and files. Your PS3 is very similar to internet explorer, and store cookies and files while you are surging the web. I hope this helps you so that you can continue to enjoy your PS3
