When document is queued to printer, using default settings,
(printer has MP tray and cassette 1)
the prompt on printer says:

USB CU Plain (first what does that mean)

Also, what should the default settings say in the menu when a cassette 1 and MP tray are used.

USB-interface; CU- custom format of paper; Plain- paper type.
Default settings in the menu of the printer is: A4 or letter,plain
That is the same thing in the printer driver. Also check format of paper settings in small window of your paper cassette.Turn roller to adjust proper size of paper.
