Puk/PIN code is a secutity code that protects your SIM card, it is unique eight digit code specific to your SIM card. This code is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. You are then given 10 attempts to enter your PUK code. If an incorrect PUK code is entered after the 10th attempt, you will block your SIM card permanently and will require a new SIM card from your mobile phone provider. PUK codes can be obtained through the following ways - The PUK code is in your SIM card documentation Starter Pack.PUK stands for Personal Unblocking Key.
You can contact to your mobile phone provider, along with documentation or you total identification, and soon after they could give the PUK code.

The PUK code is in your SIM card documentation Starter Pack.If you do not have this then -Please contact your phone service provider. Your cell phone service provider can give you the PUK code once you give themyour account details and they have confirmed your identity.
However IF it is to do with your PUK code see below..
PUKs, or personal unblocking keys, are used to unblock a mobile phone which becomes blocked. Mobile phones may become blocked when a PIN (personal identification number) is entered incorrectly on three consecutive occasions. In some instances, the phone may become blocked, which requires a PUK to unblock it, and in other instances, the SIM (Subscriber Identity Module) card itself becomes blocked and again will require a PUK to unblock it. Mobile phones are also blocked when they are reported stolen and if there is an outstanding payment on the account. It is important to differentiate between unblocking phones (using a PUK) and unlocking mobile phones (using another type of code). NOTE: It is a criminal offence for anyone to attempt to unblock a mobile phone, other than the network provider who put the block on the phone. If your phone has become blocked by mistake, for example you have entered the PIN incorrectly on three consecutive occasions, then you need to contact the mobile phone network operator with your details. To explain a little more, specific to your mobile phone network, You need to go to your Service providers, web site & navigate your way to the specific PUK details for your mobile phone network operator. Usually you have to Register your phone to get a PUK code.
The SIM card protects your subscription and not the phone from unauthorised users. If you change SIM cards you can still use your phone with a new SIM card. Most SIM cards are blocked at the time of purchase. If your SIM card lock is on you have to enter your PIN (Personal Identity Card) every time you turn on the phone.
If you enter the wrong password three times in a row the SIM card gets blocked. This is indicated by the message PIN Blocked To unblock this you need to enter your PUK Note: card (Personal Unblocking Key). Your PIN and PUK is supplied by your service provider. You can edit your PIN and select a new four to eight digit number
If the Message Codes do not match appears when you edit your PIN you entered the new PIN incorrectly.
If the message Wrong PIN followed by Old PIN appears then it means that you have entered the old PIN incorrectly.
To unblock your SIM card:
When PIN Blocked appears enter your PUK > OK
Enter a new four to eight PIN > OK
Re-enter the new PIN to confirm > OK
To edit your PIN:
> Settings > the General tab > Security > Locks > SIM Protection > Change PIN
Enter your PIN > OK
Enter a new four to eight digit PIN to confirm > OK
To turn the SIM card On /Off
> Settings > the General tab > Security > Locks > SIM Protection > Protection and select On or Off
It means the puk has been entered in wrong more than 3 times and you must now get a new sim card
Typically when a phone is asking for a PUK code it means you have tried to edit or set the secuirty pin code. Unfortunately these codes are already predefined by the sim card and the service provider. It gives you the sensation that you can set or change the code but you cannot. You get 3 attempts to enter the correct code after which the phone asks for a PUK code. When it asks for the PUK you also get three tries to undo it. Again PUK codes are predefined by the sim card and the service provider. If you call your service providers technical support number or customer service number they can provide you with the PUK or the PIN code.
>>>> Once the PUK has been entered in wrong 3 times the sim card is shot and deactivated. At this point you will need to get a new sim card to get your phone up and running again. If you have numbers stored to your sim card and not to your phone you will have lost them.
