The PUK code can be found in the packaging of the sim card when you first bought it. Several wrong tries for a pin code can get the sim card blocked and only the PUK code can enable it for access. If you can't find the PUK code, you have to buy another sim card or if this is postpaid, you have to report it to your Telecom Operator.
If you enter wrong password for more then 5 times,it ll ask you to enter the puk number....now call your customer care and get the puk number...enter the puk number in the space given for password....thats it...problem is fixed If you had locked your mobile then update the formware all saved passwords will be deleted.
OR if you contact your customer care they will unlock from there.
The PUK code is a result of entering the wrong sim pin to many times. Just call your provider or log on their website and they will give you a PUK Code. It's a security measure, and a pain in the neck. I know AT&T has a PUK request on their website.
