You need to call the customer care of the service provider... no need do anything and nothing can help you on this since the PUK Code is related to the network not to the phone.. Your PUK code is saved on the Service Provider's System so just call them by dialing the toll free customer number or service number.. and tell them that you are prompted to enter the puk code, they will take few informations from you, but don't hesitate.. just provide what they are asking and they will tell you the number and precautionary measure not to continue again.. But as soon as your phone is unlocked, they will change the PUK and your current PUK code will be invalid at the same time. Note: don't try to do something foolish by entering the puk code again and again your sim will be rejected when u reach the limit (most service provider has four times entry limit) So be careful.. or you'll never get back your sim..
