Hi the only place you can get the puk code for your phone is from the network the phone is from you can sometimes get it off their web site depending on what network but if you phone them up they will give you the 11 digit code over the phone for free.
You have to get it from your network provider as its has different code for each network.
What network is the phone on and ill try and get the code for you if you are having trouble locating it ok give me a shout back

To get your PUK code, if your phone is using a SIM Card, Check the SIM card Pack and you will see your PUK code there. But if you don't have a SIM card pack, you can get the PUK code from your service provider.
Call the customer care line of your service provider using another phone, request for your PUK code and it will be given to you. Note that your service provider will ask you some few questions about your line in order to be sure you are the real owner.
Note that your service provider alone is the only one that can give you your PUK code because it's only your service provider that has it.

These directions are for an AT and T phone, but other than that, are the same for any carrier. A PUK (PIN Unlock Key) code is required to unlock SIM cards that have become locked following three successive incorrect PIN entries. If your SIM card becomes locked, you will need to get a PUK code from your phone carrier to unlock your SIM card. You can obtain your phone's PUK code from your carrier in one of these ways: Log in to Your account.online Use a Technical Support Chat Contact your carriers Customer Care Get Your Phone's PUK Code Online Go to the Phone/Device page. From the My Phone/Device screen, select the Unblock SIM Card link under the My Make & Model heading. The Unblock SIM screen will display your PUK code and will give you instructions for unblocking your SIM Card. Once you receive your PUK code, you will need to enter it into your phone to set a new PIN code. Note: If you enter the wrong PUK code 10 times in a row, your SIM card will be invalidated, and you will need to purchase a new SIM Card.
