Basically sir, PUK code is not asked by Cellphones, rather, by SIM cards, your SIM required 4 digit PIN code and someone entered it 3 times incorrectly and now its asking for PUK code and your service provider (means your network company like Vodafone, ROGERS etc) will give you 8 digit code (mostly free) and if you'll enter PUK 3 times incorrectly then you'll lose your SIM card and data in it.
You can get the PUK code only from your Mobile service provider. Call the customer care through a friends phone to get you puk code.

If your handset asks for PUK code, you need to contact the service provider and provide them 20 digit sim number for the PUK code .
Service provider numbers will be toll free.
Once you get the PUK code, enter it . And then it will ask for new PIN number, you need to enter 0000 twice.

call the customer care which sim card u r using, they will ask u a number which is available on sim card u have to tell them that number and ask for puk code and reseting code.
best way, to get the puk code you best contact your network provider who will give you a puk free to unlock your mobile this will then unlock your your phone.
