You will need your PUK1 and PUK2 code to unlock it, it is not because of the phone, it is the line itself or the simcard.

Go & take it to the company your phone is with. Usually, the puk code is on the packet that your sim card came with. Because you kept messing with the puk code, your sim is now blocked. You may have to get a new sim card, but its best to do some web searchinq or go & take it to Vodafone (if thats the place your phone was bought from)
