If you have replaced the spark plugs yourself you may have cracked the ceramic on a plug... this will cause a misfire, remove all the plugs and check them very carefuly for cracks etc.. and carefuly replace them but dont drop them into the holes and dont overtighten them and start them with your hands before useing a ratchet.

I wouldn't guess. I would check the mixture if carbourated or maybe it has an issue with the injector timing, if fitted.

Starts then stops

The spark plugs may have become oil fouled. Nothing to worry about. Just replace the spark plugs, start the engine and let it idle for 10 minutes.

New head and gasket now engine smokes

On petrol engines, the presence of black smoke from exhaust indicates that the air / fuel is not correct. If we check and find the spark plugs on them a layer of black smoke, so no air is uncertain.
The fix is very simple: replace the filter element from the air filter, clean or replace the spark plugs and, if necessary, adjust the air intake flap from. Black smoke usually disappears after the first fixes (replacing the air filter and clean or replace the spark plugs can be done even by the driver.

Spitting sputtering

These things are notorious for having crank sensor failures. Not too expensive and not too dificult to replace. I would try that first.

My truck misses when it is first started and run ok when it warms up. It is a 4.3 motor automatic.

Do you get black exhaust smoke when first started? If so, then there is too much fuel at first which probably fouls several spark plugs, but as the engine warms up, the plugs start firing again. If no black smoke--is the check engine light ON? Read the code(s) if so and follow the clues.
