your operating system (Win Vista/XP..) is the cause of the problem. Some of the windows files are corrupt.
Also concerning the Password issue, have to reformat your system.
Reinstalling the system will put your system back in order. The missing/corrupt files will be solved and you will have the opportunity to disable password request,expect you still want to use password.
With this, the whole problem would be resolved. Give it a trail.
