It sounds as if the mirror/lenses are collecting some dirt. Here's the cleaning procedure if you want to try it.

If you open the top cover, there are 6 silver screws and 2 black screws that hold the top panels on.
remove the 4 silver screws closest to the front.
then pop off the right and left side covers. [start with the top center... just pull.] There is a clip in the front that you may need a small screwdriver to help pry loose. These clips are right below the dark gray panel over the scanner unit. Pry/pull away from the front of the printer.
There are 2 gray clips, 1 on either side of the scanner assembly. Insert a small screwdriver into them to pop them out. You should be able to lift the scanner assembly out of the printer now. Unplug the power to the motor and unplug the unit from the control board if you want. Then remove the 6 black screws from the bottom of the unit. There should be 3 mirrors visible now. I wouldn't recommend removing them... to avoid damaging them, and to not have the hassle of re-aligning them later. You should be able to use a glass cleaner on them, just don't spray cleaner directly onto the surface.
Once clean, just reassemble.
Hope this helps!

How do I solve the "scanner failure power off and on again" message for an h.p. 6110 officjet all-in-one printer/scanner/fax?

Hi Paul,

Scanner Failure usually results in Hardware Failure. However there is one step you can try. Try connecting the printer directly to wall outlet or any other power outlet and the issue should be resolved.

Regards,
Sean

HP Officejet 6310 scans once but then says scanner failure

hi,

that error message is nothing to do with the software part, it actaully hardware issue try the following,

1) unplug the machine and connect it to wall outlet directly,
2) unplug the power cord from the printer and press and hold ON for 1 min and release the ON button and plug it back..
3) Disconnect the power cord from the product.Press and hold # and 6 while reconnecting the power cord to the product, and then release both buttons and if printer turns off, turn it back ON and wait until it cmopletes its instialization and if still persists, it a hardware failure...............

HP OfficeJet D145 unable to print or scan

It could be the bulb. Sometimes disassembling the scanner and thoroughly cleaning the glass and mirror will help.
